Sue and colleagues report that more than 200 solid organ transplant recipients have had autochthonous HEV infection since 2006, but little is known about the burden of HEV in SOT recipients in North America. In a retrospective, single-center, cross-sectional study, these investigators assessed the prevalence and risk factors associated with HEV infection in SOT recipients at their facility. Children and adults who received allografts at Johns Hopkins Hospital between 1988 and 2012 were evaluated for signs of HEV infection by testing post-transplant serum samples for HEV antibodies by enzyme immunoassay and HEV RNA by quantitative reverse transcription polymerase chain reaction. Individuals with evidence of post-transplant HEV infection were compared to individuals without evidence of infection and evaluated for risk factors associated with infection. A total of 12 people developed HEV infection after transplantation. Post-transplant HEV infection has been associated with an increased risk of transplant rejection. No person developed a chronic infection. The authors concluded that SOT recipients in the United States are at risk for post-transplant HEV infection. Furthermore, they indicated that a prospective multicenter study is needed to complement the results of this retrospective single-center study to better understand HEV infection in SOT recipients in North America.

The overall HIV testing rate among individuals with a diagnosis suggestive of IDU was 8.6% in 2010-2016, with an increase in 2010-2013. The rate was stable in 2014 and 2015, but dropped in 2016. Men, by comparison, were less likely to be tested for HIV with women. Patients aged 2029 years had a higher rate of HIV testing compared to other age groups, only 4.6% of 4049 years were tested. Individuals residing in the Northeast and urban areas had higher HIV testing rates compared to other regions and rural areas, respectively. The more times a patient has used health services, the more likely they are to be tested for HIV. Patients screened most frequently were twice as likely to be tested for HIV compared with patients screened only 13 times a year.

The USPSTF reports that the estimated prevalence of chronic HCV infection is approximately 1.0%, with an estimated 44,700 new HCV infections occurring in the United States in 2017. The USPSTF states that cases of acute HCV infection increased by approximately 3 .8 times over the last decade due to increased injecting drug use and better surveillance, with the fastest increase in young adults aged 20 to 39 who inject drugs.

The USPSTF concluded with moderate confidence that screening adults 18 to 79 years of age for HCV infection has a significant net benefit. Most adults only need to be screened once. Individuals at ongoing risk of HCV infection should be screened regularly. There is limited information about the specific screening interval that should occur in people who remain at risk of new HCV infection or how pregnancy modifies the need for additional screening. This recommendation provides new evidence and supersedes the 2013 USPSTF recommendation that recommended screening for HCV infection in individuals at high risk of infection and single screening in adults born between 1945 and 1965. Therefore, the 2020 USPSTF recommendation extends the age for screening to all adults aged 18 to 79. According to the USPSTF, the Centers for Disease Control and Prevention is in the process of updating its HCV screening guidelines.

Electronic Health Record Screening Protocol

Where possible, the EMR provides an essential component to successful HCV screening by providing a best practice alert that notifies clinicians and staff when a patient is eligible for screening. Ideally, this alert is linked to a single HCV screening test for eligible patients with the appropriate diagnostic code. Upon completion of the test, the BPA should turn off but highlight a positive result. The most efficient test to order is an anti-HCV antibody that reacts to a quantitative HCV RNA in the same blood sample to confirm chronic HCV. Study design and data source

We conducted a retrospective study using the NHI Lab & Exam Dataset results as a reference standard and linked them to the NHI Outpatient Claims Dataset to evaluate the performance of the ICD-10-CM codes in identifying patients with HBV and HCV infections. This study was approved by the Institute of Review Board of the National Cheng Kung University Hospital with file number B-ER-107-394.

In order to reduce duplicate ordering of the same test or examination for a given patient by different physicians in different clinical settings, the Taiwanese administration of NHI developed the MediCloud system in 2016. A physician can query the MediCloud system to view laboratory test results and reports of exams performed in previous medical visits to avoid duplicate exams and exams. For persons with a diagnosis or procedure suggestive of UDI, the overall HCV testing rate was 7.7%. It increased from 2010 to 2014, plateaued in 2015, but dropped in 2016. Men were less likely to get tested for HCV compared to women. Patients aged 2029 years had a higher rate of HCV testing compared to other age groups. Individuals residing in the Northeast and urban areas had higher testing rates. Similar to HIV testing, more frequent users of health services were more likely to be tested for HCV than less frequent users.

Patients were more likely to receive an HCV test if their first clinical encounter was at an inpatient rehabilitation program, STD clinic or reproductive health center, public health facility, or hospital than at a general practice. Patients with a diagnosis of substance use or dependence were 1.5 times more likely to be tested for HCV than patients without the diagnosis. Patients diagnosed with substance abuse drug intoxication, overdose or attempted suicide, or a CPT code for MAT were more likely to be tested for HCV than patients without these conditions. Patients with an infection indicative of IDU were less likely to be tested for HCV.

What led to this new Hcv screening recommendation

New Recommendation B is an update and extension of the USPSTFsRecommendation 2013physicians to screen high-risk patients for HCV and offer single testing for adults born between 1945 and 1965. There are two reasons for extended screening:

New evidence shows a dramatic increase in the number of new HCV cases each year, now about 3.5 times higher than 10 years ago, and
Better treatments for HCV are now available, with greater safety and effectiveness.

The expansion of the HCV epidemic is largely due to the rapid increase in rates among young people, particularly women, intravenous drug users, and people living in rural areas. And HCV infections are expected to increase in people over age 79 as baby boomers age. The Advisory Statement brings to light the disturbing reality that HCV infects one-third of injecting drug users aged 18-30, that HCV prevalence in women aged 15-44 has doubled from 2006 to 2014, and that the percentage of babies born to HCV-infected mothers has increased by 68% in recent years.

New recommendations for Hcv screening

HIV ICD-10 CM codes - When to use Z-Code vs. B-Code

To update its previous recommendation, the USPSTF commissioned a systematic review of the evidence that included nearly 100 studies. The new review was similar in scope to the previous one, except that it also looked at evidence from young people. The evidence gathered led the USPSTF to issue a draft statement with a B recommendation for HCV screening in all adults aged 18 to 79 years. The task force also suggests that clinicians consider screening patients under the age of 18 and over the age of 79 for HCV if they are at high risk of infection. The Centers for Medicare and Medicaid Services recently announced Medicare coverage for hepatitis C virus screening in accordance with the United States Preventive Services Task Force's recommendations. As of June 2, 2014, CMS will cover screening for HCV with the appropriate laboratory tests approved/cleared by the US Food and Drug Administration. Tests are covered when requested by the beneficiary's GP or GP.Who is eligible?Testing must be performed by a qualified Medicare provider for adult beneficiaries who meet one of the following risk categories:

High risk of HCV infection, defined as people with a current or past history of illicit drug use and people who received blood transfusions before 1992. Annual screening will be performed for individuals who have continued to receive injections of illicit drugs since testing for HCV. previous negative dependency.

Uspstf develops new screening recommendations for Hcv

New recommendations are aimed at curbing the growing number of people infected with the hepatitis C virus. To address this public health dilemma, the US Preventive Services Task Force has issued a draft recommendation statement on screening for HCV infection in adolescents and adults and is now proposing that all adults ages 18 to 79 should be screened for HCV.
